ONCE YOU SUBMIT YOUR REGISTRATION ONLINE, YOUR RESPONSE HAS BEEN RECORDED. YOU WILL RECEIVE A COPY OF YOUR RESPONSES TO THE EMAIL ADDRESS YOU PROVIDED WHILE COMPLETING THIS FORM. PLEASE PRINT THIS OUT. THERE WILL BE NO ADDITIONAL CONFIRMATIONS. 

District approval for your professional day DOES NOT INDICATE THAT YOU HAVE BEEN REGISTERED FOR THE WORKSHOP. YOU MUST COMPLETED THE ONLINE REGISTRATION FORM after you have received approval from your district. Online registration will be closed if a workshop has met maximum capacity for that session.
